1. What is the remote patient monitoring program?


When patients are diagnosed with COVID-19, they need to be quarantined until the infection is cleared, usually for at least two weeks. Some patients will be at risk of complications and need comprehensive care at home.


The remote patient monitoring program screens the patient's vital signs and symptom assessment every day with medical technology and equipment. A nurse team of remote patient monitoring regularly monitors data and contacts patients to assess their health status and progress towards achieving health goals. 2. How does the remote patient monitoring program work?


The remote patient monitoring program has developed two tracks for COVID-19 patients: one for patients with complications at moderate to high risk, and the other for low-risk patients.


Patients participating in the high-risk program will receive a set of remote patient monitoring kits, including a blood pressure cuff, the 4G RPM Blood Pressure Cuff TeleRPM BPM Gen 2, thermometer, pulse oximeter, and scale. They use these devices two to four times a day to measure their vital signs. The results are automatically sent to the cloud platform through a cellular-enabled tablet computer received by the patient with the kit. The remote patient monitoring nurse will also call the patient to determine if they have symptoms such as nausea and tachypnea. Patients with a low risk of complications use the application to record vital signs. These patients receive notifications on their mobile devices every day reminding them to provide information to their care team through the application.
